The Patient Care Coordinator (PCC) is responsible for coordinating the delivery of medical care, as formulated by UWMC Neurosurgery physicians or their designee, and patients' access to various health care services. The PCC will facilitate patients' entry into and through the health care system, coordinate patient care services, and serve as a patient advocate. Under the general direction of the Department Chief, with assistance from the Health Service Manager, the PCC will use knowledge of the clinic, division and hospital policies/procedures, insurance coverage requirements (including pre-admission authorization and third party payer and reimbursement procedures) to coordinate all patient services such as admitting, arranging diagnostic tests or procedures, scheduling surgery and consultation requests, and scheduling services of other hospital departments to facilitate delivery of care to patients.The ideal candidate should have experience working with: *Pituitary: Collaboration with Endocrinology and ENT Support *Cranial and Skull Base Tumors *Spine: Simple/Complex/Orthopedic *Endovascular/Cerebrovascular/Stroke *Functional Neurosurgery: Deep Brain StimulationJob Duties and Performance Standards:Patient Access:*Act as primary liaison for UWMC/UW Medicine and represent Neurosurgery departmental physician(s) with the hospital, patient and family, referring physicians, outside agencies, staff from outside hospitals, and general public by providing information and advice regarding UWMC hospital services. *Work independently under the general direction of multiple assigned physicians or their designee. *Use ability to triage medical information in a specific medical or surgical specialty. *Use effective oral and written communication with a variety of healthcare professionals, patients and their advocates, hospital and clinic personnel both at UWMC and other medical facilities, representatives of insurance companies, and other organizations related to the health care industry. *Demonstrate the ability to be organized and effectively prioritize work tasks for the needs of the clinic. *Request interpreters as appropriate for appointments, admissions, and surgery. *Have proficiency using a variety of computer systems such as REG/ADT, EPIC, PHS, ORCA/Mindscape, OneHealthPort and Microsoft Products. *Maintain knowledge of billing and medical record documentation requirements, diagnostic tests and procedures and specialized pre-procedure instructions with guidelines set by the department or clinic Chairperson or Medical Director.Patient Intake/Insurance: *Answer the telephone in a professional and courteous manner and direct calls appropriately. *Responsible for reviewing registration data such as demographic, insurance, referral, next of kin, etc., and updating online or providing information to appropriate personnel so an update may be done. *May provide clinic charges for visit types, procedures, and surgeries to patients or prospective patients to the clinic. May provide estimated charges based on plan of care for purpose of patient deposits when patient is determined to be self-pay. This is done in conjunction with Financial Services and under the direction of the Health Services Manager, clinic Manager/Department Administrator.Manager/Department Administrator.*Contact review organizations and insurance companies to ensure prior approval requirements are met before patients enter the system such as pre-authorization and referrals; present necessary medical information such as history, diagnosis, prognosis, and length of stay prior to admissions; provide specific medical information to financial services to maximize reimbursement to the hospital and physicians. *Ensure timely submission of EPIC PreAdmit to allow Financial Counseling/Clearance Center sufficient time to secure pre-authorization prior to surgical procedures. *Maintain up-to-date knowledge of insurance pre-procedure approval requirements of most commonly encountered third-party payers.Coordination:*Act as a liaison between patients, consulting physician, referring physician or outside agency to coordinate patient appointments, admission, or surgery. Will coordinate the exchange of information either written or verbal before and after patients are seen. *Obtain all necessary medical records, diagnostic studies, pathology items, etc. that are needed for patient care. *Coordinate patients' entry into the system to include the availability of necessary medical records, coordination of tests, procedures and appointments with all hospital departments. *Schedule patient appointments by following the appropriate guidelines by clinic, department and physician. Scheduling: Admission*Responsible for completing an EPIC pre-admit for all planned hospital admissions or overnight stays which informs Admitting, Financial Clearance Center, and UWP Financial Counseling of the admission. *Responsible for reviewing registration data with patient for scheduled inpatient surgery/admission or outpatient surgery/procedures. E.g. demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done. *Responsible for coordination of pre-admission tests and procedures. *Coordinate the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff and admitting through the Patient Flow supervisor. *Responsible for communicating any cancellations or changes to Admitting, Financial Clearance Center, UWP, Financial Counseling and medical staff. Scheduling: Surgery*Act as liaison between patient, physicians, OR, nurses, other clinics, nursing units, admitting, Financial Counseling, blood bank, lab, Radiology, Social Work, Interpreter Services, OT, PT, Prosthetics, etc. for the purpose of coordinating all pre-op planning and preparation. *Schedule and communicate pre-op planning and preparation to patient. Sends appointment and other information to patients. *Responsible for requesting necessary surgical equipment, tools, supplies, devices, and implants, including custom implants. *Manage OR block time. Releases block time and requests elective OR time according to established policy guidelines. *Responsible for scheduling, rescheduling, and canceling surgeries in the PHS scheduling system. Will monitor case order day prior to surgery and make any necessary changes. *Responsible to communicate any cancellations or changes to the operating room, Pre-Anesthesia Clinic, neurophysiology monitoring personnel, etc. *Assist with completion of pre-surgery kit and electronic yellow packet. *Follow-up Care: May assist with discharge from clinic or hospital by providing follow-up appointments, alerting Social Work of potential need for transportation or housing.Requirements:A Bachelor's Degree in a medically related or public relations field AND two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology OR equivalent education/experience.
